Postoperative antibiotic prophylaxis in prepectoral breast reconstruction is not well-characterized. Pooja Yesantharao @pyrao1 and her coauthors at Stanford University conducted a multi-institutional investigation of the optimal duration of postoperative antibiotic prophylaxis in prepectoral patients. Among patients who are at standard risk for postoperative infection, continuing antibiotics beyond 1 day significantly reduced infection rates, but continuing antibiotics prophylaxis beyond 14 days did not confer additional clinical benefit. Among high risk patients (e.g. diabetics, smokers), however, antibiotic duration did not impact infection rates and alternative strategies for infection control require investigation.
